Services

At Resource Connect Solano, we lead Solano County’s Coordinated Entry System—a streamlined, equitable process designed to connect people experiencing homelessness to housing and supportive services. Our approach is grounded in four core elements, each intentionally built to promote equity, dignity, and access for all:

  • Access – We ensure that individuals and families across the county can easily connect to help through multiple, low-barrier entry points—whether through walk-ins, phone lines, or mobile outreach. 

  • Assessment – We use standardized, person-centered tools that are designed to identify each person’s unique strengths, challenges, and housing needs. We center equity by listening first and recognizing that experiences of homelessness are shaped by systemic barriers like racism, disability, and poverty.

  • Prioritization – With limited resources, we focus on housing those with the most urgent needs first--ensuring our system remains fair, transparent, and focused on equity. 

  • Referral – Once assessed and prioritized, participants are connected to housing and services that best match their situation, IF and WHEN a resource or placement is available. We work closely with our community partners to ensure that referrals lead to real, lasting solutions.

Our team works hand-in-hand with service providers, landlords, and people with lived experience to build a responsive, inclusive system of care-because housing is a human right, and a connection is where it begins. 

Eligibility for Resource Connect Solano

Eligibility for referral to Resource Connect Solano includes individuals and families who are sleeping in places not meant for human habitation (such as on the street, in a vehicle, or in a motel), those staying in emergency shelters or transitional housing programs, or those fleeing domestic or dating violence or human trafficking.

Access

Opening the Door to Support

Access is the first step in the Coordinated Entry process—and it’s where connection begins. At Resource Connect Solano, we ensure that people experiencing homelessness can easily reach services through multiple, low-barrier entry points across the county. Whether through walk-in centers, outreach teams, or phone-based support, our goal is to create welcoming, equitable access for everyone, because no one should struggle to find help when they need it most.

Assessments

 Understanding Each Person’s Unique Needs

Once someone connects to our system, the next step is assessment. At Resource Connect Solano, we use a standardized, person-centered approach to learn about each individual’s housing history, barriers, and strengths. Our assessments are designed to be equitable and trauma-informed, ensuring that everyone is heard, respected, and evaluated based on their real needs, not assumptions. This helps us match people to the right resources while honoring their lived experience and voice.

Prioritization

Responding to the Most Urgent Needs First

Prioritization ensures that our system remains equitable and effective, especially when resources are limited. At Resource Connect Solano, we focus on identifying and uplifting those with the most severe needs—such as people experiencing chronic homelessness, health challenges, or high barriers to housing. Guided by data, community input, and lived experience, our prioritization process helps ensure that support reaches those who need it most, first—because fairness means centering urgency and impact.

Referrals 

Connecting People to the Right Resources

Once individuals are assessed and prioritized, we move with care and coordination to connect them to housing and services that best meet their needs. At Resource Connect Solano, referrals aren’t just about matching people to programs—they’re about building trust, reducing barriers, and creating pathways to long-term stability. We work closely with providers across the county to ensure that each referral leads to real opportunity, real housing, and real change.
